Passports

• Available online or at the Community Services Hub at 418 Payne Road in Scarborough
• Must be complete in black ink
• Do not sign this form as it must be signed in person in our presence

Please bring one of the following:

• A certified birth certificate issued by the city, county, or state
  • Applications under 16 years old must submit a copy of their birth certificate AND any unexpired passport books or cards
• This must have a registrar’s raised, embossed, impressed, or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar’s office, which must be within 1 year of your birth. The full names of the applicant’s parent(s) must be listed on all certificated birth certificates
• Previously issued undamaged U.S. Passport
• Consular Report of Birth Abroad or Certification of Birth
• Naturalization Certificate
• Certificate of Citizenship

The original evidence (certificate or passport), not a copy, will be submitted with the passport application. It will eventually be returned to you under separate cover.

Please bring one of the following: (Community Services can make a photocopy of your identification to go with the passport application.) 

• Valid Maine Driver's License
• Valid Maine identification card
• Previously issued, undamaged U.S. passport
• Certificate of Naturalization with identifiable photo attached
• Current Government ID (city, state or federal)
• Current Military ID (military and dependents)

• Please do not complete until in person
• You will be paying for the passport fee and agent’s execution fee
• You may also choose to pay extra for expedited service
