Vital Records Request
The primary objective of the vital records function of the Town Clerk's Office is to obtain and preserve documentary evidence concerning births, marriages and deaths that is necessary to protect the legal rights of individuals.
Birth Information
In the State of Maine, when the birth of a child occurs, the birth certificate is filed in the municipal clerk’s office in the town where the birth happened, as well as in the municipal clerk’s office of the town of residence where the mother was living at the time of the child’s birth. For example, if a child was born at a hospital in Portland, and the mother was living in the Town of Scarborough at the time of the child’s birth, a copy of the birth certificate would be filed with the City of Portland and with the Town of Scarborough. Certified copies of the certificate can be purchased from the Clerk's Office in either municipality.
In addition, duplicate copies of all birth records are filed with the State of Maine Office of Vital Records, a division of the Department of Human Services.

Death Certificate
In the State of Maine, a death certificate is filed in the municipality in which the death occurred, as well as the municipality in which the deceased resided. Certified copies of the certificate can be purchased from the Clerk's Office in either municipality.

Marriage Information
To request a certified copy of a marriage license, please complete the application linked below or write us a letter including the following information:
- Full name of the both spouses, including middle name
- Date of marriage
- Name and mailing address where we should send the certificates
- Number of certified copies you are ordering
- Must include a photocopy of a government issued photo I.D.
- Please include a self addressed stamped #10 (business size envelope
